舌根部異所性甲状腺の1症例

A Case of Lingual Ectopic Thyroid with Hypothyroidism

I．はじめに

　異所性甲状腺は胎生期の発生学的異常により生じると考えられている^1）。その発生機序として，甲状舌管における甲状腺原基の下降障害説，あるいは下降路とは異なった部位への迷入説が挙げられている。舌盲孔部に甲状腺組織が残存した舌根部に発生する報告が最も多いことから^1,2），下降障害説が唱えられている。迷入説を裏づけるものとしては，前側頸部^3,4），心臓^5），肝門部^6），胆囊^7）などに発生を認めた報告が散見される。

　われわれが経験した舌根部異所性甲状腺の1症例を報告する。

　A 47-year-old woman complained of a lump in her throat. A smooth surface tumor at the base of the tongue was observed through a fiberscope. Thyroid function test demonstrated hypothyroidism. A 123-iodine scan revealed uptake confined to the lingual region and CT confirmed an absence of thyroid tissue in the anterior neck. After a conservative levothyroxine therapy,blood levels of the thyroid hormones were normalized and the size of tumor decreased.
