We describe a rare occurrence of Forestier disease(diffuse idiopathic skeletal hyperostosis:DISH)presenting with throat discomfort. A 69-year-old man presented with difficulty swallowing of solid foods and chronic cough. Fiberscopic examination disclosed a posterior swelling extending from the middle to the lower pharyngeal wall. The mucosal surface was smooth and flat, and both vocal cords showed good mobility. Lateral cervical radiographs showed ossifications of the anterior longitudinal ligament along the anterior cervical spine from C4-5 to C6-7. CT of the neck confirmed a massive ossification anterior to the cervical spine. Accordingly, the patient was diagnosed of Forestier disease. By his request,no treatment was undertaken, and the ossification was not changed during the observation years.
