下歯肉癌を合併した頭蓋外内頸動脈瘤の1例

A case of aneurysm of extracranial internal carotid artery with lower gingival cancer

I．はじめに

　頭蓋外の動脈瘤の中で，頸動脈瘤の占める割合は0.4％～4.9％と少ない^1,2）。頭蓋外内頸動脈瘤は非常に稀な疾患である。今回，下歯肉癌を合併した頭蓋外内頸動脈瘤症例に対し，原発巣の切除，再建，頸部郭清術と動脈瘤の切除および血管再建術を併せて行い良好な結果を得たので報告する。

Aneurysms of the extracranial internal carotid artery are relatively rare vascular lesion. A 73-year-old woman was diagnosed of the lower gingival cancer. CT scan also revealed a saccular aneurysm located at the left ICA,which was 46×23mm in size. The patient underwent resection of aneurysm and end to end anastomosis,mandibular segmental resection,neck dissection at the same time. The postoperative course was uneventful. 3DCT was considered very useful for evaluation of the form of aneurysm appropriately and less furthermore little invasively.
