We performed phacoemulsification- aspiration and implantation of two foldable acrylic intraocular lenses (IOLs) in 9 eyes of 6 patients. All the eyes had short axial lengths with the preoperatively estimated IOL power of +30 diopters or more. The piggyback IOLs were implanted into the capsular bag in all the eyes without intraoperative complica-tions. All the eyes showed significantly improved uncorrected and best-corrected visual acuity after surgery. The postoperative refraction shifted towards hyperopia in all the eyes than predicted. During 6 months after surgery, no major postoperative complications developed except slight interlenticular opacification in 3 eyes and posterior synechia in 3 eyes. The findings show the usefulness of piggyback IOL implantation in extremely short eyes. As a major problem, the current system of IOL power calculation markedly underestimates the actual IOL power.
