要約 目的:眼軸長が短い眼での白内障手術で,最適な術前検査法と眼内レンズ計算式の組合せの評価。対象と方法:過去5年間に白内障手術を行った84眼を対象とした。眼軸長は20.78~21.99mmであった。術後屈折の誤差をSRK/T式,Holladay式,Haigis式について比較した。結果:IOLマスターでは,SRK/T式またはHaigis式,AモードではHolladay式で眼内レンズの度数を計算することが,術後の屈折誤差が最小であった。どの計算式でも,AモードはIOLマスターよりも約0.7D遠視化した。結論:眼軸長が短い眼での白内障手術では,IOLマスターで計測し,SRK/T式またはHaigis式で眼内レンズの度数を計算することで最も高い精度が得られた。
Abstract. Purpose:To evaluate the accuracy of formulas for the calculation of power of intraocular lens for eyes with short axial length. Cases and Method:This retrospective study was made on 84 eyes that received cataract surgery in the past 5 years. All the eyes had axial length between 20.78 to 21.99 mm. Postoperative refraction was compared with the outcome of SRK/T,Haigis and Holladay formulas. Results:Postoperative error of refraction was the minimum when calculated by SRK/T or Haigis formulas usisng IOLMaster,and by Holladay formula using A-mode measurement. Postoperative refraction shifted towards hyperopia by 0.7 diopters using A mode than IOLMaster calculated by each of 3 formulas. Conclusion:Accuracy of calculated power of intraocular lens was the greatest by measuring with IOLMaster and calculated by SRK/T or Haigis formulas in eyes with short axial length.
