Xanthomonas maltophiliaによる眼内レンズ挿入術後眼内炎

A case of endophthalmitis due to Xanthomonas maltophilia following intraocular lens implantation

　Xanthomonas maltophiliaによる眼内レンズ挿入術後眼内炎の1例を報告した。糖尿病を有する70歳男性で，日帰り手術にて超音波水晶体乳化吸引術および眼内レンズ挿入術を施行後経過良好であったが，術後13日目から前房蓄膿を伴う眼内炎を生じた。抗生物質投与に加え，早期に眼内レンズ摘出術および硝子体切除術を行い，良好な経過を得た。術中に得られた硝子体液，前房水，摘出眼内レンズの培養検査でXanthomonas maltophiliaによる感染が証明された。本菌による眼内レンズ挿入術後眼内炎の報告は過去にない。本菌はグラム陰性菌で，時に日和見感染を起こす弱毒菌であるが，ヒビテン消毒に対し抵抗性を示す。今後高齢者や糖尿病などの全身疾患を合併する易感染性患者の術後眼内炎においては，本菌を含めた弱毒菌眼内炎を考慮する必要がある。

A 70-year-old diabetic male underwent uneventful phacoemulsification with posterior chamber lens implantation in the left eye. Acute and severe endophthalmitis with hypopyon, fibrinous exudation and vitritis developed 13 days after surgery. The eye was treated by pars plana vitrectomy and removal of intraocular lens. Xanthomonas maltophilia was identified from the vitreous and the removed lens.

Xanthomonas maltophilia is a gram-negative rod and an organism of low virulence. It is not a pathogen for normal adults. This is the first documented case of endophthalmitis by this organism after cataract surgery. Immunocomprised state seemed to be predisposing factor in this patient.
