A 70-year-old diabetic male underwent uneventful phacoemulsification with posterior chamber lens implantation in the left eye. Acute and severe endophthalmitis with hypopyon, fibrinous exudation and vitritis developed 13 days after surgery. The eye was treated by pars plana vitrectomy and removal of intraocular lens. Xanthomonas maltophilia was identified from the vitreous and the removed lens.

Xanthomonas maltophilia is a gram-negative rod and an organism of low virulence. It is not a pathogen for normal adults. This is the first documented case of endophthalmitis by this organism after cataract surgery. Immunocomprised state seemed to be predisposing factor in this patient.
