Bacillus subtilisによる眼瞼感染症と考えられた1例

A case of eyelid infection presumably by Bacillus subtilis

（25C2-11）　Bacillus subtilisによる眼瞼感染症と考えられた症例を報告した。症例は2歳，女児。左上眼瞼に発赤，腫脹および腫瘤形成がみられた。抗生物質を投与しても軽快しなかった。CTやMRIで左上眼瞼に腫瘤陰影がみられたため悪性腫瘍を疑い，腫瘤摘出術を行った。病理組織学的に悪性像はなく，肉芽腫様炎症像がみられたため何らかの感染症が疑われた。腫瘤表面からの細菌培養にてBacillus subtilisが検出されたことから，本症例は本菌による眼瞼感染症と考えた。

A 2-year-old girl presented with tumorous swelling of the left upper eyelid of 3 weeks' duration. Systemic antibiotics was futile. Computed tomography and magnetic resonance imaging showed a mass lesion in the eyelid. Surgically extirpated tumor showed granulomatous inflammatory lesion. Culture of the removed tumor showed Bacillus subtilis as the probable cause.
