持久性隆起性紅斑に周辺部角膜潰瘍を合併した3症例

Three cases of erythema elevatum diutinum with peripheal corneal ulcer

（26E-19）　周辺部角膜潰瘍を合併した持久性隆起性紅斑の3症例を経験したので報告した。症例1と症例3は両眼に発症し，点眼治療では治癒しなかったため，Brown法に準じた角膜切除術を行った。症例2は片眼に発症し，ステロイド点眼，抗生剤点眼のみで治癒した。3症例ともリウマチ因子が陽性であり，症例3は慢性関節リウマチを合併していた。持久性隆起性紅斑の治療薬であるdiamino-diphenyl sulfoneの内服により，角膜潰瘍の症状が改善した。周辺部角膜潰瘍と皮疹の合併をみた場合，全身性合併症として持久性隆起性紅斑も考慮すべきと考える。

We observed three cases of erythema elevatum diutnum with peripheral corneal ulcer. The corneal ulcer was bilateral in 2 cases. After topical medication proved futile, the eyes were successfully treated by keratectomy after Brown's procedure. The other unilateral case could be cured by topical antibiotics and corticosteroid. Rheumatoid facter was positive in all the cases. One case had a previous history of rheumatoid arthritis, and his skin and eye lesions improved by systemic diamino-diphenyl sulfone, DDS. These cases illustrate that erythema elevatum diutnum is a possibility when peripheral corneal ulcer is associated with skin lesions.
