We reviewed 4 infants of optic nerve aplasia. The condition was unilateral in 3 cases and bilateral in one. One unilateral case underwent autopsy. Funduscopy and fluorescein angiography showed the absence of optic disc and retinal vessels in all the affected eyes. The autopsied case showed absence of retinal ganglion cells and retinal vessels, and the optic nerve could be identified as a trace. Computed tomography and ultrasonography showed absence of the optic nerve. X-ray studies showed extremely narrow optic canals. The findings showed that optic nerve aplasia may have an extreme form in the same spectrum as optic nerve hypoplasia. Vitreous abnormalities were present in 4 of the affected 5 eyes. They included fibrovascular tissue, hemorrhage and retinal detachment. These vitreous malformation may derive from failure of the embryonic fissure closure and ganglion cell formation.
