（25C1-17）　視神経無形成の4乳児例を検索した。3例は片眼性，1例は両眼性であった。片眼性1例には剖検が行われた。全例で検眼鏡と螢光眼底造影で視神経乳頭と網膜血管が欠如していた。剖検では，網膜神経節細胞と網膜血管が欠如し，視神経は痕跡のみがあった。CTと超音波検査では視神経は同定されなかったが，視神経管撮影で患側の視神経管狭細があり，視神経無形成には低形成と同一スペクトルム上の非常に重篤な例があることが示唆された。患眼5眼中4眼に硝子体の形成異常を伴っており，経過中に硝子体出血や網膜剥離などの多彩な臨床像を呈した。これらの成因として胎生裂の形成異常および神経節細胞の発達障害が関与している可能性がある。

We reviewed 4 infants of optic nerve aplasia. The condition was unilateral in 3 cases and bilateral in one. One unilateral case underwent autopsy. Funduscopy and fluorescein angiography showed the absence of optic disc and retinal vessels in all the affected eyes. The autopsied case showed absence of retinal ganglion cells and retinal vessels, and the optic nerve could be identified as a trace. Computed tomography and ultrasonography showed absence of the optic nerve. X-ray studies showed extremely narrow optic canals. The findings showed that optic nerve aplasia may have an extreme form in the same spectrum as optic nerve hypoplasia. Vitreous abnormalities were present in 4 of the affected 5 eyes. They included fibrovascular tissue, hemorrhage and retinal detachment. These vitreous malformation may derive from failure of the embryonic fissure closure and ganglion cell formation.
