研修病院における白内障手術教育

Training of cataract surgery in a teaching hospital

　当院では，1992年から白内障手術初心者に対し，1眼目から超音波水晶体乳化吸引術を教育し，最初の症例から全手術過程を執刀させることを目標としてきた。8名の初心者が当院で1,027眼の手術を行った。術中合併症は48眼（4.7％）に生じた。合併症の種類は，後嚢破損42眼（87.5％），チン小帯断裂6眼（12.5％）であった。前嚢破損は全症例中101眼（9.8％）に生じた。これは合併症例48眼のなかで28眼に生じ，58.0％を占めていた。アンケートにおいて研修医は合併症を起こさない要点として，術眼の把握，使用機器・器異の完全な準備，術者と患者の姿勢を慎重に決めることの3点が大切であると答えている。このことは，術前の準備が何よりも重要であるという，当院の手術に対する姿勢が受け継がれているものと思われた。

We have been training aspiring ophthalmic surgeons for cataract surgery. They were first trained in phacoemulsification and aspiration (PEA). After having mastered this stage, they later perform full course of PEA. Eight surgeons have performed PEA in 1,027 eyes. Intrasurgical complications developed in 48 eyes (4.7%) , including posterior capsule rupture 42 eyes (88%) and partial rupture of zonules 6 eyes (13%). There were 101 instances of anterior capsule rupture which included 28 of the above complications. The trainees reported three major items in preventing complications : better understanding of the eye to be operated, due setup of surgical instruments and apparatus, and ideal posturing of the patient and the surgeon during surgery.
