Abstract. Purpose:To report complications during cataract surgery in eyes with or without pseudoexfoliation(PEX)syndrome. Cases:Cataract surgery was performed on 9,609 eyes during 6 years through 2006 in our hospital. The series comprised 603 eyes with PEX syndrome and 9,006 eyes without. Results:Zonular rupture occurred in 3.6% of eyes with PEX and in 0.3% of eyes without. Uneventful phacoemulsification-aspiration could be performed in 92.4% of eyes with PEX and in 97.6% of eyes without. Intraocular complication in eyes with PEX was more frequent in the presence of hard nucleus and poor mydriasis. Conclusion:Zonular rupture during cataract surgery was 12 times more frequent in eyes with PEX than in eyes without. Posterior capsular rupture was 1.8 times more frequent in eyes with PEX. PEX syndrome is a risk factor during cataract surgery,particularly in eyes with hard nucleus or poor mydriasis.
