網膜剥離手術後の網膜下液吸収遅延

Delayed absorption of subretinal fluid after scleral buckling procedures

　裂孔原性網膜剥離に対し手術を施行した213眼について，網膜下液吸収に要した期間と吸収遅延の原因について検討した。213眼中23眼（10.8％）において，手術後網膜下液がすべて吸収されるのに4週間以上を要した。網膜下液吸収遅延には，術前の網膜下沈着物と色素の有無，手術時網膜下液排出が完全に行えたか否かが関係しており，裂孔の種類，デマルケーションラインの有無，手術回数に関係はなかった。また，年齢，屈折度，水晶体の有無，網膜剥離の範囲にも関係はなかった。網膜下沈着物の有無は網膜下液吸収に大きな関係があり，術式の選択や再手術施行の決定のさいに参考とする要因の1つであると考えた。

We reviewed the postoperative course of rheg-matogenous retinal detachment in 213 eyes duringthe foregoing 8-year period. All the eyes underwentscleral buckling procedure with cryo-orphotocoagulation. The subretinal fluid was drainedintraoperatively whenever possible. In 23 eyes, 11%,absorption of subretinal fluid took longer than 4weeks. Delay in subretinal fluid absorption waspositively correlated with the presence of sub-retinal precipitates (χ²-test, p＜0.01) or pigmentand incomplete drainage (p＜0.025). It was nega-tively correlated with the patient's age, refractiveerror, presence of aphakia, area of retinal detach-ment, characteristics of retinal breaks, presence ofdemarcation lines and the number of prior opera-tions. The present finding shows the importance ofsubretinal precipitates in deciding the mode ofsurgery and in assessing the postsurgical outcome.
