体位変換によって網膜下液が移動する非裂孔原性網膜剥離

Non-rhgematogenous retinal detachment with shifting subretinal fluid

　体位変換によって網膜下液が著明に移動する非裂孔原性網膜剥離の17症例を，検眼鏡的に限局性色素上皮病巣の有無によって2群に分類した。その結果，後極部に限局性の色素上皮病巣の認められるI群11例は，宇山の報告した後極部多発性色素上皮症およびGassの報告した胞状網膜剥離に相当するものであった。そして，同一症例に両者の病巣が認められたものが3例存在したので，両者は同一の疾患であり，表現型が異なるものであろうと考えた。また，検限鏡的には後極部に限局性の色素上皮病巣の認められないII群　6例は，臨床的にはSchepcnsの報告したuveal effusionに類似していたが，originalの報告では螢光眼底造影所見の記載がないため，同一の疾患であるという根拠は確認できなかった。しかしII群では，一時期，脈絡膜からの高度な螢光色素の漏出が認められるので，網膜剥離の原因として一過性に脈絡膜網膜関門のbarrierの機能的障害が生ずると考えた。

　治療は，I群では光凝固が有効であり，II群では予後は不良であった。

We evaluated a consecutive series of 17 cases with non-rhegmatogenous retinal detachment showing shifting subretinal fluid seen during the past 9 years. These cases were divided into two groups : Group I showing REP lesions in the posterior fun-dus (11 cases) and Group II showing no apparent RPE lesions (6 cases).

The funduscopic features of Group I were com-patible with bullous retinal detachment (Gass) or multifocal posterior pigment epitheliopathy (Uyama). Both lesions were present in the same subject in 3 cases : a feature which suggests that both diseases analogous in nature with slightly dif-ferent clinical features.
