糖尿病網膜症の螢光眼底像に対する画像処理—毛細血管瘤について

Image processing of fluorescein angiograms of early diabetic retinopathy

　軽症糖尿病網膜症の螢光眼底像にコンピューター画像処理を行い，毛細血管瘤およびその前段階の病変の検出に有用かどうか検討した。IMAGEnet^®を使用し，contrast enhancement処理を行い，強調された点状過螢光について検討した。その結果，1）不鮮明な螢光眼底写真では，過半数からオリジナルでは読影困難な毛細血管瘤を検出することが可能であった。2）鮮明な螢光眼底写真では，毛細血管瘤の前段階の病変を観察しやすくできることが確認できた。3）背景螢光の一部が強調されて，毛細血管瘤とまぎらわしい螢光を呈する場合もみられた。本結果について，今後の課題も含めて考察した。

We applied computerized image processing to fluorescein angiograms of early diabetic retinopathy using IMAGEnet by Topcon. The processing showed, frequently, dot hyperfluorescen-ce which we named possible MA. Comparison of the processed images with routine angiograms ena-bled a better identification of true microaneurysms (MAs). The present procedure also enabled detec-tion of MAs in unclear angiograms. It allowed detection of localized abnormalities in retinal capil-laries or pre-MA status. Some granular back-ground hyperfluorescence was occasionally mis-taken for MAs.
