　1988年，1989年の2年間に硝子体手術を行った糖尿病性網膜症102例，136眼の手術成績を1983年，1984年の2年間の手術成績と比較検討を行った。

　1）視力向上を得たのは網膜剥離を伴わない群で89％，網膜剥離を伴う群で67％，全体で73％であった。

　2）術後視力0.1以上を得たのは網膜剥離を伴わない群で77％，網膜剥離を伴う群で55％であった。

　3）眼内レーザー光凝固の導入と，周辺部硝子体の処置により，術後の血管新生緑内障と網膜剥離の発生率は大幅に低下した。

　4）術後に血管新生緑内障を発生した症例の予後は今回の検討でもいまだ不良であり，今後さらに検討を要する課題である。

We reviewed the results of pars plana vitrectomy in 136 eyes with severe intraocular complications of diabetic retinopathy during the foregoing 2-year period. Visual acuity improved in 99 out of 136 eyes, 73%. The series included 101 eyes with retinal detachment and 35 eyes without at the time of surgery. Visual acuity improved in 68 eyes, 67%, in the former group and in 31 eyes, 89%, in the latter.Final visual acuity better than 0.1 was obtained in 56 eyes, 55%, in the former group and in 27 eyes, 77%, in the latter. Above results were superior to those of ours from earlier period. The improved outcome is ascribed to the use of intraocular laser photocoagulation, aggressive removal of peripheral vitreous, and exclusion of cases with longstanding macular detachment from indication for surgery.
