硝子体手術長期術後成績に関する臨床的研究—1．非シリンオイル注入眼の検討

Long-term visual results of vitreous surgery without using silicone oil

　硝子体手術症例の長期術後成績を，疾患別に比較検討した。対象は1986年までの6年間に新潟大学眼科において硝子体手術を施行し，シリコンオイルを注入しなかった147眼である。生命表理論を用いて検定した結果，網膜静脈閉塞症や網膜前膜・ぶどう膜炎・Terson症候群等は術後3か月から1年の視力を5年以上維持できること，網膜剥離や穿孔性眼外傷は術後3年まで網膜剥離の発生・再発のため視力低下の可能性があること，糖尿病網膜症は術後5年経過しても視力低下の可能性があり，原因の多くは網膜機能の低下であることが判明した。

Using Kaplan-Meier method, long-term visual results in the first 147 consecutively vitrectomized eyes in ophthalmic clinic of Niigata University Hospital from 1978 to 1986 were evaluated. The percentages of patients whose visual acuity were better than preoperative and better than 0.01 at 3 years after surgery were: 38.6% in diabetics, 70.1% in vein occlusions, 50.8% in retinal detachments, 36.1% in perforating injuries, 67.7% in others (uveitis, pre-retinal membrane, Terson). Major complications for poor prognosis were retinal detachments, glaucomas and dysfunctions of the retina.
