We treated 11 eyes of submacular hematoma by injecting pure sulfur hexafuoride gas (SF6) with the aim to displace hematoma from the fovea. After treatment, the hematoma was displaced from the fovea in 3 eyes. The hematoma decreased in the other 8 eyes. Displacement of hematoma was greater in eyes with larger amount of hematoma. The causative lesion could be identified in 5 eyes (46%) prior to treatment. It was identified in all the 11 eyes after treatment comprising age-related macular degeneration 10 eyes and polypoidal choroidal vasculopathy 1 eye. After displacement of hematoma, the initially hidden choroidal neovascularization (CNV)was detected in 6 eyes of age-related macular degeneration by fluorescein and ICG angiography. The CNV remained unchanged in extent in 3 eyes and its whole extent could not be detected in 1 eye. Polypoidal choroidal vasculopathy was diagnosed by the presence of racemose CNV. There was no untoward effect of gas injection throughout the cases. After treatment, the visual acuity improved by 2 lines or more in 7 eyes, remained unchanged in 3, and deteriorated in 1. The findings show that intervitreal gas injection is safe and effective for submacular hematoma and that it may facilitate angiographic detection of underlysing CNV.
