加齢黄斑変性に生じた黄斑下血腫に対するt-PA網膜下注入による血腫移動術の効果

Outcome of subretinal injection of tissue plasminogen activator to displace subretinal hematoma in age-related macular degeneration

要約　目的：加齢黄斑変性（AMD）に伴う黄斑下血腫に対するt-PAの網膜下注入と硝子体ガス置換を併用した血腫移動術の効果を検討する。

対象と方法：対象は，AMDに伴う黄斑下血腫に対し，t-PAの網膜下注入と硝子体ガス置換による硝子体手術を施行した15例15眼である。硝子体切除後に38G網膜下注入針を用いてt-PA約0.1mlを黄斑下血腫部位に直接注入した後，硝子体ガス置換を行った。

結果：15眼中13眼（87％）で黄斑下血腫は良好に中心窩から移動し，視力と中心網膜厚は有意に改善した。

結論：AMDに伴う黄斑下血腫に対するt-PAの網膜下注入と硝子体ガス置換を併用した血腫移動術は有効である。

Abstract　Purpose: To report the outcome of vitrectomy followed by subetinal injection of tissue plasminogen activator（t-PA）and gas tamponade in eyes with age-related macular degeneration（AMD）.

Cases and Method: This study was made on 15 eyes with submacular hematoma secondary to AMD. All the eyes received vitrectomy with subretinal injection of 0.1 ml t-PA followed by gas tamponade.

Results: Submacular hematoma was displaced away from the fovea in 13 out of 15 treated eyes（87％）. These cases showed significant improvements in visual acuity and central retinal thickness during the ensuing 6 months.

Conclusion: Vitrectomy with subretinal injection of t-PA was effective in displacing submacular hematoma away from the fovea and resulted in improved visual acuity and decreased central retinal thickness.
