Two patients of rheumatoid arthritis presented with severe ocular manifestations. The first case, a 64-year-old woman, showed central corneal perforatoin in the right eye and corneal ulceration in the left. Her poor systemiccondition did not allow immunosuppressive therapy nor keratoplasty. The second case, a 57-year-old woman, showedscleritis, vitreous opacity, exudative retinal detachment and white mass in the peripheral fundus in both eyes. The ocular lesions responded well to systemic prednisolone and methotrexate with topical betamethasone and cyclosporine. Severeocular manifestations of rheumatoid arthritis may require topical or systemic immunosuppressive therapy as they mayreflect potentially lethal systemic vasculitis and as they are difficult to respond to conventional topical medications.
