自己免疫性視神経炎の3例

Three cases of autoimmune optic neuritis

要約　目的：自己免疫性疾患に併発した視神経炎3症例の報告。症例と経過：症例はいずれも女性で，年齢は18歳，50歳，50歳であった。1例は全身性エリテマトーデスの経過中に右視神経炎，6か月後に左視神経炎が発症し，ステロイドとシクロホスファミドのパルス療法で軽快した。他の2例は視神経炎を繰り返し，経過中に中枢神経症状を伴う原発性Sjögren症候群と診断された。ともにステロイドのパルス療法が奏効したが視野異常が残った。ステロイドの投与期間が短かったことが再発ないし視野異常が残った原因と推定された。結論：視神経炎では自己免疫性疾患が関与していることがある。ステロイドパルス療法が奏効することがあるが，減量に留意する必要がある。

Abstract.　Purpose：To report three cases of autoimmune optic neuritis. Cases and Findings：All were females aged 18,50 and 50 years respectively. One case had systemic lupus erythematosus followed by optic neuritis in the right eye,and 6 months later,in the left. She improved after pulsed treatment with systemic corticosteroid and cyclophosphamide. The other two cases showed recurrent optic neuritis followed by manifestations of Sjögren syndrome with involvement of central nervous system. Pulsed treatment with corticosteroid led to improvement with partially impaired visual field. Conclusion：Autoimmune disease may underlie optic neuritis. Method of tapering is important in pulsed corticosteroid therapy.
