増殖性糖尿病網膜症18眼と網膜静脈分枝閉塞症6眼に硝子体手術を行い,硝子体と血液中のレプチン濃度を測定した。全24眼に硝子体出血が発症していた。硝子体手術を行った黄斑円孔または網膜上膜22眼を対照とした。硝子体中のレプチン濃度(pg/ml)の平均±標準誤差は,糖尿病群1,162±291,網膜静脈分枝閉塞症群528±124であり,対照群での90±29よりも有意に高かった。糖尿病群と網膜静脈分枝閉塞症群の間には有意差がなかった。血中レプチン濃度には,3群間に有意差がなかった。硝子体出血が併発している糖尿病網膜症と網膜静脈分枝閉塞症では,硝子体中のレプチン濃度が増加していることと,レプチンが直接または間接に眼内血管新生に関与する可能性があると結論される。
We performed vitreous surgery on 18 eyes of proliferative diabetic retinopathy and 6 eyes of branch retinal vein occlusion. All the 24 eyes had vitreous hemorrhage. We quantitated the concentration of leptin in the vitreous and the circulating blood. Another series of 22 eyes of macular hole or epiretinal membrane served as control. The intravitreal concentration of leptin averaged 1,162±291pg/ml in diabetic eyes and 528±124pg/ml in retinal vein occlusion. These values were significantly higher than 90±29pg/ml in controls. There was no significant difference between diabetic eyes and retinal vein occlusion. There were no significant differences in leptin concentration in the blood among the three groups. The findings show that intravitreal leptin concentration is elevated in eyes of proliferative diabetic retinopathy and 6 eyes of branch retinal vein occlusion associated with vitreous hemorrhage. It appears that leptin is involved,either directly or indirectly,in intraocular angiogenesis.
