腎移植後慢性拒絶反応の治療中に発症したサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis in a case during treatment for chronic rejection after renal transplantation

46歳男性が20年前に囊胞腎に対して腎移植を母親から受けた。4年前から慢性拒絶反応が起こり，副腎皮質ステロイドと免疫抑制薬の投与を受けていた。両眼の霧視が6か月前からあり，ぶどう膜炎として紹介受診した。矯正視力は右0.3，左0.8であった。両眼に前房の炎症細胞，硝子体混濁，眼底後極部を中心とする顆粒状白斑と静脈の白鞘化があり，右眼眼底の上方に広範な白色病巣があった。前房水からサイトメガロウイルスDNAが検出され，サイトメガロウイルス網膜炎と診断した。それまで使用中の副腎皮質ステロイドと免疫抑制薬を減量し，ガンシクロビルの投与で網膜炎は軽快したが，腎機能が悪化し透析が必要になった。腎移植後の日和見感染症としてのサイトメガロウイルス網膜炎の治療が困難であることと，他科との連携が必要であることを示した症例である。

A 46-year-old male received renal transplantation for renal cyst from his mother 20 years before. Chronic rejection developed 4 years before,necessitating use of corticosteroid and immunosuppressive agents. He was referred to us for blurring in both eyes since 6 months before. His corrected visual acuity was 0.3 right and 0.8 left. Both eyes showed cells in the anterior chamber,vitreous opacity,sheathing of retinal veins,and white patches in the retina. An extensive white lesion was present in the superior quadrant of the right fundus. Cytomegalovirus（CMV）DNA was isolated from the aqueous,leading to the diagnosis of CMV retinitis. He was treated with systemic ganciclovir and reduction of corticosteroid and immunosuppressants. Ocular lesions disappeared,but hemodialysis had to be started due to aggravation of renal rejection. This case manifested problems in the treatment of CMV retinitis occurring as opportunistic injection.
