太陽性網膜炎の1例

A case of solar retinopathy

　精神分裂病の男性に見られた太陽性網膜炎を報告した。症例は42歳男性で，精神分裂病にて11年間にわたり投薬治療中であった。1991年12月中旬，正午から2時間太陽を凝視していたところ，両眼に中心暗点と視力低下が出現した。

　太陽性網膜炎は，日食を不十分な防護手段で観察して発生することが多い。日食以外の条件下で発生する太陽性網膜炎の報告は少なく，精神病により太陽を凝視して発生したものは稀である。この症例は，5か月経過した時点でも視力は両眼とも0.2と低下したままであり，眼底所見では中心窩に1／10乳頭径程度の黄斑円孔様の病変部が存在しており，その周囲には，網膜の浮腫・色調の変化は見られなかった。以上の所見より，太陽性網膜炎のなかでも重篤で不可逆な症例である。

A 42-year-old male had been under medical treatment for schizophrenia. He developed central scotoma with visual impairment in both eyes after he gazed at the sun for 2 hours from noon in mid-December. He manifested foveal lesion simulating a macular hole of one-tenth disc diameter in botheyes. The retial area around the lesion was free of edema or dispigmentation. The visual acuity 5 months after the episode remained 0.2 in either eye.The case illustrates that gazing at the sun for 2 hours may result in serious and irreversible damage to the fovea.
