We reviewed 28 cases who were operated for paralytic strabismus secondary to cerebrovascular disorders during the past 16 years. The series comprised 15 cases of cerebral ischemia and 13 cases of cerebral hemorrhage or infarction. The ocular motility disorder was identified as supranuclear palsy 10 cases, oculomotor palsy 8 cases, trochlear palsy 6 cases, abducens palsy 3 cases, and multiple palsy 1 case. Systemically, there were 15 cases (54%) of systemic hypertension and 6 cases (21%) of diabetes mellitus. Functional cure was obtained in 16 cases (57%) after strabismus surgery. The other 12 cases without surgical success included 7 cases of supranuclear palsy, probably due to poor fusional function or impaired motor innervation to the eye.
