新しい癌化学療法を試みた涙腺癌の1例

A case of lacrimal gland adenocarcinoma treated by chemotherapy

（C2-1-4）　骨浸潤のみられた涙腺癌を摘出後，新しい癌化学療法の併用を試み，術後2年間再発がみられない1症例を経験した。症例は53歳男性。Hertel眼球突出計にて左眼26mmの眼球突出がみられた。CT,MRI検査にて左眼窩深部まで達する腫瘍が観察され，腫瘍生検により涙腺癌との病理診断を得た。左眼窩内容除去術を行い，術中病理診断にて周辺眼窩骨組織への腫瘍浸潤所見がみられたので，可及的に除去し．術後化学療法を行った。5-fluorouracil（5-FU）の24時間点滴とシスプラチン（cisplatin：CDDP,5mg／m²）の隔日少量投与の併用療法を行い，2年後の現在まで再発所見を得ていない。本例においては，5-FUとCDDPの新しい組み合わせと投与方法の工夫による化学療法が．骨浸潤のあった涙腺癌に対して奏効し，本法の有効性が推定されたが，今後も経過観察を要するものと考えられる。

A 53-year-old male presented with exophthalmos in his left eye since 6 months before. A tumor reaching the posterior orbit was detected by computed tomography and magnetic resonance imaging. Biopsy led to the diagnosis of lacrimal gland adenocarcinoma. Infiltration of tumor into the bony orbit was found during exenteration of the left orbit. After surgery, he was treated by continuous drip infusion of 5-fluorouracil and daily cisplatin at the dosis of 5mg/m². He has been doing well until 2 years after surgery. This case illustrates the efficacy of our new combined chemotherapy.
