A 48-year-old woman presented with panuveitis, vitreous hemorrhage, choroidal tumor and severe exudative retinal lesions in her left eye. The findings simulated masquerade syndrome or malignant lymphoma. The condition was finally diagnosed as posterior scleritis after detection of typical thickening of the posterior sclera by computed tomography and magnetic resonance imaging. Treatment with topical and systemic corticosteroids induced improvement in subjective and objective findings with normalization if scleral thickness. The affected eye developed secondary glaucoma and complicated cataract. Good visual acuity and intraocular pressure control could be achieved following trabeculectomy with mitomycin C, phacoemulsification-aspiration and intraocular lens implantation.
