A 67-year-old female presented with blurring in both eyes since 6 months before. The intraocular pressure (TOP) was 20 mmHg right and 24 mmHg left. Neither eye showed conjunctival injection, cells or flare in the anterior chamber, or vitreous opacity. Mild cataract was present in both eyes. Gonioscopy showed white nodules over the trabecular meshwork in the left eye. Topical corticosteroid induced normalization of IOP. It appeared that the elevated IOP was due to inflammation in the trabecular meshwork, possibly by sarcoidosis. This case shows the importance of gonioscopy in differentiating primary glaucoma from secondary ones.
