内頸動脈閉塞症に続発する緑内障—Ⅱ．血管外科不適応例に対する検討

Rubeotic glaucoma secondary to internal carotid artery occlusion (ICAO): Ⅱ. Prevention of iris rubeosis

　内頸動脈閉塞症に続発する虹彩ルベオーシスが，trabeculectomyまたは，汎光凝固術によって消失した。

　trabeculectomyすることにより，眼圧が下降し眼動脈圧と眼圧との圧差が拡大し，眼内循環が改善したと考えられた。また，汎光凝固は網膜を間引くことにより，hypoxiaに対応していると考えられた。

　これらの手段により，続発緑内障の発生を予防できた。

A 64-year-old male developed acute ischemic reti-nopathy in the left eye due to ipsilateral internal carotid artery occlusion. Diabetes mellitus had been detected 1 year before. Rubeosis iridis became manifest 7 months after the ischemic retinal epi-sode. Trabeculectomy resulted in normalization of ocular tension, decrease of extravasation from re-tinal vessels and shortening of arm-to-retina circu-lation time from presurgical 20 seconds to post-surgical 17 seconds.
