超音波生体顕微鏡で検出したアトピー性皮膚炎患者での高頻度の毛様体裂孔

Detection of ciliary epithelial breaks at a high incidence in atopic patients by ultrasound biomicroscopy

　眼科に紹介されたアトピー性皮膚炎患者15症例30眼に対し，超音波生体顕微鏡（UBM）で毛様体と眼底の最周辺部を検索した。網膜剥離は8眼（27％），裂孔は13眼（43％）にあった。13眼の裂孔のうち8眼（62％）はUBMで初めて発見された。裂孔の位置は10眼が毛様体（皺襞部6眼，扁平部4眼）で，3眼が硝子体基底部であった。眼症状を自覚しない9例18眼では，6眼（33％）に裂孔がUBMにより発見された。アトピー性皮膚炎では，自覚症状がなくても裂孔の頻度はかなり高いと思われる。これらの多くは毛様体裂孔であり，検眼鏡では発見しにくい。本疾愚ではUBMの診断的価値が高い。

We examined 30 eyes of 15 patients who were referred to us for atopic dermatitis. Retinal detachment was present in 8 eyes (27%) and retinal or ciliary breaks in 13 eyes (43%). Ultrasound biomicroscopy (UBM) led to the detection of breaks in 8 of these 13 eyes (62%). Breaks were detected by UBM in 6 (33%) of 18 asymptomatic eyes. The breaks were located in the vitreous base in 3 eyes, in the pars plana in 4 eyes and pars plicata in 6 eyes. The findings illustrate the usefulness of ultrasound biomicroscopy in detecting ciliary or retinal breaks in eyes of atopic patients.
