A 43-year-old male presented with herpes zoster in the area of left trigeminal nerve. He had been under treatment with systemic immunosuppressants and corticosteroid for acute renal failure secondary to polyarteritis nodosa. Herpetic iritis developed in the left eye 5 months later. The right eye developed numerous exudates and narrowing of arteries in the retina another 20 days later. The right eye was diagnosed as acute retinal necrosis secondary to herpes zoster ophthalmicus in the left. Retinal detachment developed in the right eye 2 months later necessitating vitreous surgery. Vitreous aspirates and the serum showed elevated titers for herpes zoster. This case illustrates that acute retinal necrosis may develop following herpes zoster ophthalmicus in the contralateral eye when the patient is in immunosuppressive state.
