We evaluated the additive or replacement effect of isopropyl unoprostone ophthalmic solution in 149 eyes of 75 patients with glaucoma or ocular hypertension. The ages averaged 66.3 years. Unoprostone instillation replaced prior beta-blocker in 26 eyes (group 1) . Unoprostone was added to beta-blocker in 73 eyes (group 2) . Unoprostone was added to beta-blocker and pilocarpine in 22 eyes (group 3) . Unoprostone was added to beta-blocker, pilocarpine and dipivefrin in 28 eyes (group 4). Significant hypotensive effect was noted in group 2 (p = 0.001) and group 4 (p= 0.004) . In eyes with initial intraocular pressure of 18mmHg or over,hypotensive effect was also noted in group 1 (p =0.001) and group 3 (p=0.015). The added incidence of effective cases by 1mmHg or more and of very effective ones by 3 mmHg or more was 62% for group 1,73% for group 2, 64% for group 3 and 64% for gruop 4. Superficial punctate keratopathy due to unoprostone developed in 13 eyes (9%) . The instillation had to be discontinued in one eye. The findings show the synergeistic hypotensive effect of topical unoprostone and beta-blocker.
