A 72-year-old male presented with bilateral cor-neal opacity and epithelial keratitis. He had been treated for glaucoma with topical instillation of 4 kinds of ophthalmic solutions during the past 4 years. Discontinuation of all medications resulted, 3 weeks later, in marked improvement of corneal lesions. Initiation of topical treatment with beta-blocker and artificial tears with preservatives wasfollowed by exacerbation of epithelial keratitis. The inflammation disappeared after application of artificial tears without preservatives alone. Ir-reversible histological corneal damage remained showing goblet cells in the corneal epithelium and disappearance of palisades of Vogt. Most ophthal-mic solutions used by this patient contained benzal-konium chloride as preservative, which appeared to be responsible for the observed corneal damage.
