A 81-year-old male presented with bilateral central scotoma and hypacusia of 3 weeks' duration.Slitlamp biomicroscopy showed keratic precipitates and cells in the anterior chamber. Papilledema and diffuse retinal detachment were present in the posterior fundus. Fluorescein angiography showed hyperfluorescence of the optic disc. Indocyanine green angiography showed findings suggesting disturbed circulation in the choroid. Pleocytosis was present in the cerebrospinal fluid. He was positive for HLA-DR4.These findings led to the diagnosis of Harada's disease. Systemic treatment with massive prednisolone induced rapid improvement in fundus findings and visual acuity. This case is, to our best knowledge, the oldest patient of Harada's disease reported so far. Spinal fluid examination and HLA typing were facilitated early diagnosis of the disease.
