外傷を契機として先天性網膜動静脈吻合症に漿液性網膜剥離を生じた1例

Serous detachment of the macula following blunt trauma in a case of congenital arteriovenous communications

　先天性網膜動静脈吻合症（Archer分類1群）で頭部打撲による衝撃が誘因となって動静脈吻合部の血管から血管外漏出をきたし，黄斑部の漿液性網膜剥離を生じた症例を経験した。症例は19歳の女性で，頭部の打撲後4日目に右眼中心暗点発生。右眼眼底には乳頭中央部から蛇行した網膜動脈が黄斑部へ向かい，細動脈を介して網膜静脈と吻合していた。黄斑部には3乳頭径大の漿液性網膜剥離を伴っていた。螢光眼底造影により，動静脈吻合が証明され，吻合部の静脈壁から旺盛な螢光の血管外漏出がみられた。1週間後には自然治癒した。本例では螢光造影の走査型レーザー検眼鏡によって観察し血流動態と本病変の発症機序が詳しく理解された。

A 19-year-old female noted central scotoma in the right eye 4 days after concussion to her forehead. Funduscopy showed serous detachment of the macula and arteriovenous communications with marked venous dilatation and tortuosity. Fluorescein angiography showed arteriovenous communications and dye leakage from the anastomosed vessel wall. The serous detachment of the macula spontaneously disappeared one week later with recovery of visual acuity from the initial 0.7 to 1.2. The blunt head trauma appeared to have triggered temporary decompensation of retinal circulation involving the macula.
