脈絡膜欠損に水晶体脱臼と網膜剥離を合併した1例

Lens luxation and retinal detachment in a case of choroidal coloboma

　鈍的外傷が原因で水晶体脱臼を生じた後に，裂孔原性網膜剥離を併発した脈絡膜欠損の1例を報告した。症例は42歳男性で，網膜剥離に対して硝子体手術を行った。術中所見として，脈絡膜欠損部に隣接した耳側網膜に膜状の硝子体癒着があり，同部位に子午線方向のスリット状の裂孔を認めた。脱臼水晶体はこの硝子体膜上に浮遊しており，周囲の網膜を間接的に牽引していた。手術は，経毛様体扁平部水晶体切除，硝子体切除，経強膜冷凍凝固，輪状締結，ガスタンポナーデを行い網膜の復位を得た。本症例では，脱臼水晶体による異常な硝子体牽引が裂孔形成，網膜剥離発症に関与している可能性が示唆された。

A 42-year-old male presented with choroidal coloboma, lens luxation and rhegmatogenous retinal detachment 2 days after blunt trauma to his right eye. During pars plana lensectomy and vitrectomy, a radial retinal break with adherent vitreous veil was detected along the temporal margin of choroidal coloboma. The luxated lens was located in apposition to the vitreous veil and exerted indirect traction to the underlying retina. The retina was reattached after additional transscleral cryopexy, scleral encircling and gas tamponade. Vitreous traction following blunt trauma appeared to have caused retinal break and detachment.
