CHARGE associationに伴う脈絡膜欠損に合併した網膜剥離の治療例

Treatment for retinal detachment in a case with choroidal coloboma and CHARGE association

　先天性脈絡膜欠損は網膜剥離が高率に発症し，一般の網膜剥離に比べて難治性である。今回筆者らは，先天性脈絡膜欠損に合併した網膜剥離に対して硝子体手術を行った1例を経験した。

　患者は14歳女性。黄斑部に隣接し視神経乳頭を含む脈絡膜欠損を右眼底下方に認め，その中央やや上方に裂孔が存在し，脈絡膜欠損を越えて眼底全範囲にわたる扁平な網膜剥離を認めた。左眼に脈絡膜欠損，小角膜を認めた。また本症例は，脈絡膜欠損の他に全身的な先天的合併症も認められ，CHARGEassociationと診断した。右網膜剥離に対して硝子体切除術を行った。裂孔から網膜下液を排液し，脈絡膜欠損部外周に光凝固を施行し，輪状締結術シリコーンオイル注入を行った。10か月後，シリコーンオイルを除去した。現在，網膜剥離は認められず経過観察中である。

A 14- year-old girl presented with total retinal detachment with typical choroidal coloboma in her right eye. A retinal break was located in the colobomatous area. She also manifested clinical features including: patent ductus arteriosus, retarded growth, meantal retardation, anomaly in the central nervous system, hypacusia and hypoplasia of auricles. The retina could be reattached by vitrectomy, drainage of subretinal fluid through the retinal break, endophotocoagulation, silicone oil tamponade and scleral encir-cling.
