Abstract Purpose:To report a case of choroidal nevus with polypoidal choroidal vasculopathy(PCV).

Case:A 65-year-old woman presented with metamorphopsia in the right eye since 4 months before.

Findings:Corrected visual acuity was 0.1 right and 1.2 left. The right eye showed flat gray area adjoined by an orange lesion upper nasal to the macula. Additional findings by angiography and fundus imaging led to the diagnosis of choroidal nevus associated with PCV. The right eye was treated 6 sessions of vitreal injection of ranibizumab and 14 sessions of aflibercept after each remission. The right visual acuity has been maintained at 0.6 for 3 years until present.

Conclusion:PCV in the present case may have been induced by choroidal circulatory disorder secondary to choroidal nevus. Intravitreal injection of anti-VEFG drugs in multiple sessions was effective in preventing exacerbations and in preserving the vision.
