We observed three eyes which developed accumulation of liquid material between the implanted intraocular lens (IOL) and the posterior capsule 12 to 40 months after cataract surgery. All the eyes were diabetic and had posterior subcapsular cataract. There were no findings suggesting inflammation in the anterior or posterior ocular segment. The anterior chamber depth and refraction remained unchanged throughout the postoperative period. The presence of liquid accumulation posterior to the IOL did not affect the visual acuity or intraocular pressure. The optical features of the liquid material, as estimated by Scheimpflug slit imaging, were more pronounced in eyes with larger distance between the IOL and the posterior capsule.
