眼内レンズ表面散乱光のためにレンズを摘出交換した症例の検討

要約　目的：眼内レンズ（IOL）表面からの散乱光のために，交換を必要とした症例の報告。対象と方法：過去3年間に霧視などのためにIOLの摘出と交換が行われた10例11眼を対象とし，診療録の記録を解析した。結果：11眼すべてにAcrySof^®（Alcon社）IOLが用いられていた。背景疾患として，5眼に糖尿病網膜症，それぞれ2眼に網膜剝離と落屑症候群があった。最初のIOL挿入時には，9眼が無水晶体眼であった。IOL挿入から交換までに4～11年，平均8.2年が経過していた。交換したIOL固定は，縫着5眼，囊外4眼，囊内2眼であった。術後の視力と角膜内皮細胞には有意な変化はなく，霧視は改善し，全例が満足した。結論：挿入したIOL表面からの散乱光があり，霧視が生じた症例にIOL交換を行い，全例で効果があった。

Abstract.　Purpose：To present a review of cases who received exchange of intraocular lens（IOL）for glistening of the surface. Cases：This retrospective study was made on 11 eyes of 10 cases who received IOL exchange in the past 3 years for blurring. Results：All the 11 eyes had been implanted with AcrySof^® IOL. Five cases had diabetic retinopathy and 2 cases had retinal detachment and exfoliation syndrome respectively. Nine eyes were aphakic at the time of initial IOL insertion. IOL was exchanged after 4 to 11 years, average 8.2 years, after initial insertion. IOL after exchange was fixated by suturing in 5 eyes, extracapsularly in 4 eyes, and intracapsularly in 2 eyes. There was no change in visual acuity or population of corneal endothelial cells after exchange. All the cases expressed great satisfaction after IOL exchange. Conclusion：IOL exchange was effective in eyes with blurring due to glistening of IOL surface.
