A 77-year-old male had undergone uneventful phacoemulsification-aspiration with posterior chamber lens implantation in both eyes 7 years before. His left eye developed unusual secondary cataract. The intraocular lens (IOL) was completely fixed intracapsularly. The anterior capsule adhered to the anterior surface of IOL forming a closed space. The space between the IOL and the posterior capsule contained milky white liquid and granules. The removed IOL and the capsule showed, histopathologically, still active lens epithelial cells in the equator. The lens fibers became degenerated and turned into structureless protein in their inner aspects. It appeared that the residual lens epithelial cells proliferated and degenerated to form the observed liquefied and granular opacity.
