内因性ぶどう膜炎併発白内障の眼内レンズ移植

Intraocular lens implantation in cataractous eyes with endogenous uveitis

　内因性ぶどう膜炎が非活動性となった併発白内障18例21眼に対し，水晶体嚢外摘出術と後房レンズ移植術を行った。臨床経過の観察は，細隙灯顕微鏡などによる眼科一般検査とともに前房フレアを定量的に測定した。術直前および術後1年目以内の経過で前房フレア値は老人性白内障の対照群に比べ高い値であったが，重篤な炎症の再燃はみられず，全例で視力の改善を得た。ぶどう膜炎の既往があっても，症例の選択と術中操作，そして術前後の観察を慎重に行うことにより眼内レンズの移植が可能と考えられる。

We performed extracapsular cataract extraction and posterior chamber lens implantation in 21 eyes, 18 patients, with inactive or controlled endogenous uveitis. Postoperative aqueous flare intensity, as measured by laser flare-cell meter, showed signifi-cantly elevated values than in pseudophakic eyes after senile cataract during the postoperative 12-month period. The differences were not clinically manifest. We advocate intraocular lens implanta-tion in inactive and well-controlled uveitis because of infrequent intra- and postoperative complica-tions, better postoperative vision and no apparent recurrences.
