We reviewed 6 cases who developed diplopia following small-incision cataract surgery under instillation anesthesia. Cataract surgery had been performed bilaterally in 5 cases and unilaterally in 1. All the cases showed vertical diplopia. Oblique diplopia was also present in 2 cases. Further examinations led to the diagnosis of suspected superior oblique paresis in 3 cases. These cases illustrate that asymptomatic superior oblique palsy may become manifest following cataract surgery, particularly in aged persons.
