目的:白内障手術時にトリアムシノロンを前房に注入したときの,術後炎症の軽減効果と副作用の検討。対象と方法:両眼に白内障手術を行った27名54眼を対象とした。全身疾患がない健康人11名と糖尿病患者16名である。それぞれ片眼にトリアムシノロン3.2mgを注入し,他眼にBSSのみを注入して対照とした。術後3か月まで経過を追った。結果:糖尿病者では,前房のフレア値がトリアムシノロンの注入眼では非注入眼よりも有意に低かった(p<0.05)。矯正視力,眼圧,角膜内皮細胞減少率,中心窩網膜厚の変化については両群間に有意差がなかった。結論:糖尿病者での白内障手術では,術中にトリアムシノロンを前房内に注入することで,術後早期の前房のフレア値の軽減が期待できる。
Purpose:To evaluate triamcinolone injection into the anterior chamber during cataract surgery regarding postoperative inflammation and complications. Cases and Method:This prospective study involved 54 eyes of 27 patients who received bilateral cataract surgery. The series comprised 11 otherwise healthy persons and 16 diabetics. Each patient received triamcinolone injection into the anterior chamber in one eye and biological saline solution(BSS)in the fellow eye. Patients were followed up for 3 months after surgery. Results:Intensity of aqueous flare one week after surgery was significantly lower in triamcinolone-treated eye than in the fellow eye in diabetic patients. There were no differences between the two groups regarding postoperative visual acuity,intraocular pressure,loss of corneal endothelial cells,or thickness of the fovea. Conclusion:Injection of triamcinolone into the anterior chamber during surgery may lessen inflammation during early postoperative period in diabetics.
