Abstract Purpose:In the 1990s, the prevalence of diplopia after cataract surgery was 2-3%. However, in recent year's cataract surgery has become less invasive. We postulated that the prevalence of postoperative diplopia has changed and investigated the occurrence of diplopia after cataract surgery.

Subjects and methods:The subjects were 3017 patients who underwent lens reconstruction surgery under topical anesthesia from July 2014 to June 2016 at Kitasato University Hospital. We retrospectively analyzed results of the alternate prism cover test at distance.

Results:Thirty-one cases(1.03%)had diplopia after surgery. Fifteen patients(0.50%)had diplopia before surgery and 16 patients(0.53%)developed diplopia after surgery. In the preoperative diplopia group, there was no significant difference in the angle of deviation before and after the operation. In the postoperative diplopia group, 7 cases were cases with intial strabismus, and 9 cases were patients who did not clearly have preoperative strabismus.

Conclusion:The prevalence of diplopia after cataract surgery in this study was lower than that previously reported. Among them, more than half of the cases complained of diplopia for the first time after the surgery. It is necessary to investigate the history of strabismus and treatment before cataract surgery. However, there were some cases in whom post-cataract surgery could not be predicted before the surgery.
