YAGレーザー後発白内障切開術後に眼内炎がみられた1例

A case of endophthalmitis following YAG laser posterior capsulotomy

（P3-1-34）　YAGレーザー後発白内障切開術後に眼内炎がみられた1例を経験した。症例は74歳女性で，水晶体超音波乳化吸引術と眼内レンズ挿入術を行い，術後経過は良好であった。しかし，後発白内障が進行してきたため，手術から1年7か月後にYAGレーザー後発白内障切開術を行った。その2日後，前眼部を主体とした眼内炎を発症した。抗生物質およびステロイド薬の局所・全身投与により，炎症は40日後に消退し再燃を認めていない。臨床経過から，初回手術時に侵入し水晶体嚢で生息していた弱毒菌感染による眼内炎が疑われた。その際YAGレーザー使用が引き金となったものと思われ，慎重な対応が望まれる。

A 74-year-old female underwent uneventful cataract surgery by phacoemulsification-aspiration and intraocular lens implantation in her right eye. At 19 months after surgery, the eye received Nd : YAG laser posterior capsulotomy totalling 58 shots for secondary cataract. Endophthalmitis mainly involving the anterior segment developed 2 days later. Topical and systemic treatment with antibiotics and corticosteroid led to cure of endophthalmitis 40 days later. The clinical course was suggestive of persistent subclinical infection by weak bacterial organism in the lens capsule since the initial surgery which became manifest after YAG laser capsulotomy.
