幼児にみられたFisher症候群の1例

Fisher syndrome in an infant

（17-P3-4）　2歳女児で，失調性歩行を初発症状とし，全方向の眼球運動障害，両側の眼瞼下垂，深部腱反射消失を認め，Fisher症候群と診断された1例を経験した。プレドニゾロンの経口投与にて運動失調，眼瞼下垂，外眼筋麻痺は2か月以内に順に回復した。以後経過良好であるが，深部腱反射は発症後1年を経過した時点でも出現していない。3歳未満のFisher症候群の報告例は少ないが，眼症状が先行し眼科を初診する可能性もあることから，幼少児の眼瞼下垂，眼筋麻痺をみた場合，本症候群も鑑別診断の1つとして考慮し，全身検索や小児科紹介の必要があると思われた。

We diagnosed a 2-year-old girl as Fisher syndrome. She manifested ataxia of gait, areflexia, restricted eye movement in all directions and bilateral blepharoptosis. Peroral prednisolone at the daily dosis of 25 mg resulted in complete improvement of ataxia followed by improved blepharoptosis and ophthalmoplegia within the following 2 months. Areflexia was still present when examined one year later. This case illustrates that Fisher syndrome may develop in early childhood well before the age of 3 years.
