We diagnosed a 2-year-old girl as Fisher syndrome. She manifested ataxia of gait, areflexia, restricted eye movement in all directions and bilateral blepharoptosis. Peroral prednisolone at the daily dosis of 25 mg resulted in complete improvement of ataxia followed by improved blepharoptosis and ophthalmoplegia within the following 2 months. Areflexia was still present when examined one year later. This case illustrates that Fisher syndrome may develop in early childhood well before the age of 3 years.
