Q-switched Nd-YAG laserの眼科臨床応用—レーザー後発白内障切開術と合併症

Q-switched Nd-YAG laser in clinical ophthalmology.:Laser discission of secondary cataract and its complications

　Q-switched Nd-YAG lascrによる後発白内障切開術を21例22眼に施行し，十分な切開とともに大多数の症例で視力の改善を得た。

　重篤な合併症として眼圧上昇が認められ，その背景因子として緑内障治療歴と残存水晶体皮質が関与していることが示唆され，このような条件を持つ症例に対しては，レーザー後発白内障切開術施行に際し，十分な注意が必要であると考える。また，上記因子を持たない症例においても，高度の眼圧上昇を来たす場合もあり，術後の眼圧モニターは不可欠である。

Q-switched Nd-YAG laser treatment was perform-ed on 22 eyes with secondary or after cataract. Adequate discission of the membrane could be ob-tained in all the eyes associated with improvement in vision in the majority of cases.

As a serious complication, some cases responded with an acute and persistent elevation of intraocular pressure. Past history of glaucoma and residual lens cortex were identified as major predictive factors for this complication. These factors ought to be evaluated when planning laser discission of secon-dary cataract. A careful monitoring of intraocular pressure is advocated after YAG-laser capsulotomy.
