Indications and limitations of capsule tension ring during cataract surgery

（C1-2-22）　白内障手術に際し，水晶体嚢の赤道部に挿入し，嚢の緊張を保つことのできる手術補助器具であるMocher社製Capsule Tension RingをZinn小帯断裂症例14例に用いた。11例はリング挿入，眼内レンズ愛内固定が可能であった。180度以上Zinn小帯が断裂し，水晶体の動揺が激しく，リング挿入が不可能であった2例と，リング挿入はできたが，核が大きく固かったためリングの効果が不十分であった1例に水晶体全摘出を行った。180度以上Zinn小帯が断裂し，水晶体が大きく傾いてしまえばリング挿入は不可能であり，術式の変更が必要となると考えられた。リングは挿入できない症例や挿入できても効果が不十分な症例もあり限界はあるが，挿入できた症例に関しては非常に有用であった。

We used a capsule tension ring in 14 eyes which showed ruptured zonule of Zinn either before or after cataract surgery. The ring could be successfully inserted in 11 eyes followed by intraocular lens insertion. Intracapsular cataract extraction became necessary in the other 3 eyes. Zonular tear over 180 degrees was present in 2 of the 3 eyes accompanied by violent oscillation of the lens and prevented insertion of the ring. Large solid nucleus was present in the other 1 eye hindering with the efficacy of the ring. The findings show the usefulness of capsule tension ring in eyes with ruptured zonule of limited extent.
