両眼性糖尿病視神経症

A case of bilateral diabetic optic neuropathy

（P1-1-16）　厳格な治療開始の2か月後に．両眼の視神経乳頭腫脹をきたした症例を報告した。症例は58歳男性，当初右眼視力低下は一過性で自然緩解した。急激な血糖是正で低酸素状態になった組織による乳頭の浮腫と微小循環障害，すなわち糖尿病乳頭症（DP）と診断した。しかし徐々にフリッカー値は低下し，左眼は無症候性乳頭浮腫の2か月後，急激な視機能低下をきたした。持続する浮腫は循環不全と軸索流障害を助長し，生じた虚血が浮腫を作る悪循環があったと推察し，前部虚血性視神経症（AlON）と診断した。両疾患は同一線上にあり，糖尿病患者における狭義のAlONをDPと分類することはできず．本症例は両者の中央に位置する病態を呈したと考えられた。

A 58-year-old male presented with blurring of vision in his right eye. Strict blood sugar control-over 5% drop of HbAic within a month-resulted in asymptomatic optic disc edema 2 months after start of diabetic treatment. The patient showed spontaneous relief of right visual acuity, so the case was first diagnosed as diabetic papillopathy (DP). Visual acuity and critical fusion frequency of both eyes suddenly deteriorated 2 month later. Both eyes showed hyperemia and swelling of the optic disc and they were diagnosed as anterior ishemic optic neuropathy (AION). AION and DP are closely related to each other. It has come to a conclusion that this case is in between the two.
